– You are a First Nations member who was removed from your family home as a child (under 18) by child welfare services between April 1, 1991, and March 31, 2022. At the time of the removal, your family home was located in a community or in the Yukon.

– You are a parent or grandparent who was the primary caregiver of a child removed from your home by child welfare services between April 1, 1991, and March 31, 2022, while you were living in a community or in the Yukon.
